Liver Resection for Metastatic Colon Carcinoma
نویسنده
چکیده
In an investigation of the indications for hepatic resection in the treatment of colorectal carcinoma metastases, the records of 859 patients who had undergone this procedure were reviewed. This patient group, from 24 institutions, was found to have a 5-year actuarial survival of33% and a 5-year actuarial disease-free survival of21%. The only factors that might by themselves be considered contraindications to hepatic resection are the presence of positive hepatic nodes, the presence of resectable extrahepatic metastases, or the presence offour or more metastases. Other factors that had a negative effect on long-term survival were margins of resection on the liver metastases less than or equal to 1 cm (S [5-year actuarial survival] 23%), the presence of positive mesenteric nodes in the primary tumor specimen (S 23%), and a disease-free interval of less than 1 year (S 24%). The effect of any one of these factors was not great enough to contraindicate resection. However, combinations of prognostic factors must be considered before resection is recommended. The overall 5-year survival rate for this large series has been very satisfying. Decision making in the future must take into account such factors as numbers of metastases, extrahepatic involvement, and stage of the primary tumor.

Colon carcinoma metastases with extension to chest wall and pericardium
Management of locally advanced colon cancer metastases is challenging and operation carries an increased risk of morbidity and mortality. The knowledge of exact sites of involvement can aid the surgeon to make a decision regarding complete resection or palliative therapy. PET/CT scan is an advantage in this setting because metabolic alterations often precede the anatomic changes; moreover, it c...
متن کاملEarly Start of Chemotherapy after Resection of Brain Metastasis from Colon Cancer with Synchronous Multiple Liver Metastases
Brain metastasis (BM) is infrequent in colorectal cancer (CRC) patients. Although BM from CRC is a late-stage phenomenon with an extremely poor prognosis, some subsets of patients would benefit from a multidisciplinary management strategy. The prognosis of patients with BM from CRC is associated with the curability of the therapy for BM and number of metastatic organs. The start of chemotherapy...
متن کاملSimultaneous hepatic and pulmonary resection for metastatic colonic carcinoma under thoraco-laparotomy with right oblique incision: case report.
To achieve complete resection of metastatic colonic carcinoma in the liver and lung, thoracolaparotomy-assisted simultaneous resection was attempted in a 60-year-old male patient who had previously undergone sigmoidectomy for primary sigmoid colon carcinoma. A solitary liver metastasis was observed in segment 7 and a solitary lung metastasis was located in segment 6 of the right lower lung. Sim...
متن کاملMetachronous metastatic colonic small cell carcinoma and recurrent adenocarcinoma in a patient with ulcerative colitis
Colonic small cell carcinomas (SCCs) are rare, aggressive tumors characterized by early metastasis and poor prognosis. We describe a 39-year-old man with a history of ulcerative colitis and left colon adenocarcinoma resected at an outside institution who presented with hematochezia and proctalgia. Work-up revealed left colon SCC with liver metastases. After his excellent response to neoadjuvant...

We report herein the case of 70-year-old woman in whom colon cancer and a synchronous metastatic liver tumour were successfully resected laparoscopically. The tumours were treated in two stages. Both postoperative courses were uneventful, and there has been no recurrence during the 8 months since the second procedure.
متن کاملSuccessful liver resection for the unusual case of peripheral intra bile duct growth of liver metastasis from colorectal carcinoma
Peripheral intrabiliary liver metastases (PILM) from colorectal carcinoma are rare, and this feature, which resembles primary cholangiocarcinoma, potentially misleads the accurate diagnosis and subsequent surgical treatment. A 67-year-old man with a medical history of descending colon carcinoma demonstrated an abnormal rise in CA19-9. There was a strong possibility of peripheral cholangiocarcin...
